Brief OT assessment to screen strengths & areas of need
Service Description
This assessment explores sensory processing & sensory regulation, fine motor skills, gross motor skills, self-care skills & participation. This assessment includes: 30 minute information gathering phone call with parents/ school, information gathering forms (completed by home & school), 1-hour school visit including classroom observation and written summary outlining strengths, areas of need & recommendations that school & home can embed. This assessment provides an overview of occupational therapy needs and helps identify whether further targeted or comprehensive assessment is recommended. Cancellation Policy
To re-arrange or cancel in-person assessment or intervention at least 48 hours notice from the start of the session must be provided to ensure a full refund is issued. To re-arrange or cancel any other OT support such as staff training, at least 48 hours notice from the start of the session must be provided. Assessments require preparation and administration in advance, including the review of information and issuing of assessment forms. Cancellations made more than 48 hours before the assessment may be eligible for a partial refund: - where an assessment is cancelled within 48 hours, or after assessment materials and forms have been issued, 50% of the assessment fee will be retained to cover administration and preparation costs & the remaining balance will be refunded where applicable) - where an assessment is rescheduled within 48 hours, an alternative date & time will be provided to complete the assessment).
