Service Description
This assessment identifies sensory processing & sensory regulation, fine motor skills, gross motor skills, self-care skills & participation. This assessment is best for identified specific concerns impacting upon school participation. This assessment includes: 30 minute information gathering phone call with parents/ school, information gathering forms (completed by home & school), 2-hour school visit (including 45 minute direct assessment session consisting of clinical observations) and written summary outlining strengths, areas of need & recommendations that school & home can embed. This assessment provides assessment into occupational therapy needs and helps identify recommendations with practical school-based strategies. (Only available in Cambridgeshire, Bedfordshire, Hertfordshire & Suffolk- please note, when booking this assessment using the calendar link, you are booking the time slot of the school visit).
Cancellation Policy
Where an assessment has been paid for and is cancelled 48 hours prior to the in person visit, a 15% administration fee for the total assessment cost will normally apply. The remaining balance will be refunded where appropriate. This is due to the administrative work (information gathering) which is completed before the in person assessment takes place. Where an assessment has been paid for and cancelled with less than 48 hours prior to the in person visit, a 30% administration fee for the total assessment cost will normally apply if the assessment is no longer taking place. If the school/ in person visit needs to be rearranged, the Occupational Therapist will offer different appointment times and attempt to book this in with the parent/ guardian/ school, charging no fee for the first missed visit. If the next arranged visit is subsequently cancelled, the Occupational Therapist will charge a 30% administration fee for the total assessment cost. The Occupational Therapist may be waived/ reduced at my discretion in exceptional circumstances. I understand that attending school/ appointments may be difficult for some children, therefore I encourage parents to let me know if they feel this may be an issue on the day of the assessment.
